Report an Incident

To report an incident, please complete the submission form by clicking on the button below. Press the "Submit" button when complete. Anyone can contribute an incident to RISI. You do not need to be a RISI member.

Submitted incidents will be investigated and sensitive information (if provided) will be removed to protect the confidentiality of the parties involved. We never publish the identity of the person submitting the incident. We only publish the name of the company affected by the incident if incident information is also available in the public domain (e.g. published in a newspaper or online).

If the incident you provide meets our submission criteria, it will be entered into the database, and you will be issued a credit for 1 month of access to the RISI database.

Submitters are not required to provide their contact information and we disable email and IP address collection. Of course, we can only issue a credit if you provide your contact information.
